An Emergency Number is VERY important! If there is a travel delay or any kind of emergency we must be able to reach the contact person the day or night before the scheduled performance in order to reschedule or adjust to the emergency. This is even MORE IMPORTANT if the program is scheduled for a Monday but there is an emergency over the weekend. We must be able to reach you!

Please submit one form for each day booked at your location. IE: if you have booked "New American" for 3/1 and 3/2 please fill out this form for 3/1 then submit another form for 3/2.

Do not schedule four consecutive presentations in a row without a break. There must be a minimum 20-30 break between two of the four shows.

Directions should be from the nearest interstate/highway or from the performers lodging if you know where that will be. (Western Washington: From Seattle or From Capitol Hill) Please include exit numbers, correct street names, and possible delays (construction, etc.)
